Absorption failure and certain medicines. Vitamin B12 in food is bound to protein and needs stomach acid plus intrinsic factor, a protein made by gastric parietal cells, to be absorbed in the small intestine. Break any link in that chain and diet stops mattering.

Four situations account for most non-dietary cases. Autoimmune destruction of intrinsic factor production, which causes pernicious anaemia.
